Better quality palliative care

When a loved one requires palliative care, the emotional load on a family is exceptional. While preparing mentally and practically for the inevitable, the focus is on providing as comfortable and pain-free a quality of life for your loved one as possible.

 

In many cases, this is best achieved in an in-home setting, with the patient in a familiar environment and visits by family and friends made easier and safer...especially given recent COVID outbreaks, hospital resource shortages and an overstrained healthcare system.

 

With In-Home Assisted Living, special attention is given to the quality of our in-home palliative care teams. Each case is assigned a team leader who will work with trained palliative care nurse professionals, your palliative physician, and Personal Support Workers (PSW) chosen for their expertise and compassionate natures. Your team will work closely with you to:

  • Act as a liason between patient, family, and the patient's healthcare team
  • Work, on your behalf, with your local Home and Community Care Support Services 
    (CCAC/LHIN) organization to take full advantage of government-funded services.

 

In-Home Assisted Living palliative care teams work collaboratively with palliative physician specialists and palliative nurses. We can assist with the necessary medical equipment required to ensure your loved one's comfort and dignity. When necessary, we can also assist with enhanced palliative care in a hospital setting or specialized palliative care facility outside of the home.
